2 min read

Being organized before your tax appointment makes your return faster and more accurate. Use this checklist to gather what you need, and skip anything that doesn't apply to you.

Personal information

  • Photo ID for you and your spouse

  • Social Security numbers or ITINs for you, your spouse and your dependents

  • Dates of birth for everyone on the return

  • A copy of last year's federal and state tax returns

  • Bank routing and account numbers if you want your refund by direct deposit

Income documents

  • Form W-2 from each employer

  • Forms 1099-NEC, 1099-K and 1099-MISC for self-employment or gig income

  • Forms 1099-INT and 1099-DIV for interest and dividends

  • Form 1099-B for sales of stocks and other investments

  • Form 1099-R for retirement account distributions

  • Form SSA-1099 for Social Security benefits

  • Form 1099-G for unemployment benefits or state tax refunds

  • Records of any other income, such as rental income

If you are self-employed

  • A summary of your business income, including payments that were not reported on a 1099

  • Receipts and records for business expenses

  • A mileage log if you use your vehicle for business

  • Dates and amounts of any estimated tax payments you made

  • Information about your home office, if you have one

Deductions and credits

  • Form 1098 for mortgage interest, and records of property taxes you paid

  • Receipts for charitable donations

  • Form 1098-T for tuition and Form 1098-E for student loan interest

  • Your child care provider's name, address and tax ID number, and the amounts you paid

  • Form 1095-A if you had health insurance through the Health Insurance Marketplace

Accounts outside the United States

If you have bank or financial accounts outside the U.S., tell us. Depending on your balances, you may have additional reporting requirements, such as the FBAR (FinCEN Form 114) or Form 8938. We will flag this during your consultation and refer you to a specialist when needed.

How to share your documents

Please don't send Social Security numbers or tax documents by regular email. When you schedule your appointment, we explain how to share your documents securely.

Ready to get started? Call 571-245-2001 to schedule your tax appointment. We work in English and Persian (Farsi).

This checklist is general information, not tax advice for your specific situation.
